Registration Forms
Once you have met with the Childcare Director, download and complete the following information and then bring it to the Edgemoor Community Center Main Office (please do not email forms).
- 2009-2010 Registration Form (Microsoft Word DOC)
- Income Eligibility Form (Adobe PDF)
Health Appraisal Form
If your child does not have a current health appraisal on file with ECC (less than two years), please download and complete the following form:

School Permission Form
If your child attends schools other than Mount Pleasant Elementary or Harlan Middle School, please complete the following:
